29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): Employee(s) using powered industrial trucks in the production area and the warehouse located at 15612 S. Keeler Terrace, Olathe, Kansas, were exposed to struck-by hazards due to the following deficiencies not being restored to safe operating conditions: (a) Truck number 7366V, outer sheath of mast hydraulic cable was damaged. (b) Truck number 1550Y, outer sheath of mast hydraulic cable was damaged. (c) Truck number 154V, hood and amber light not working. (d) Truck number 25781Z, left side door lower glass panel had been impacted resulting in cracks in the glass.
