A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. a. Bedrock Earthworks, LLC located at or near 6470 North Hamilton Road, Westerville, OH 43081: On or about February 26, 2026, the employer failed to ensure that employees performing work in a trench that varied in depth between 5 feet 1 inch and 5 feet 7 inches were provided with a safe means of egress, exposing the employees to caught-in and struck-by hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: a. Bedrock Earthworks, LLC located at or near 6470 North Hamilton Road, Westerville, OH 43081: On or about February 26, 2026, the employer failed to ensure that a protective system was in place to prevent collapse of the faces of the trench while employees were performing work in a trench that varied in depth between 5 feet 1 inch and 5 feet 7 inches, exposing the employees to caught-in and struck-by hazards.
