29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a) Job site, 1427 North Franklin Street, Pittsburgh, PA: On or about February 26, 2026 during box gutter restoration activities, the employer did not ensure an employee working on a scaffold was provided scaffold training. Deficiencies noted included, but were not limited to, not having proper fall protection in use, failing to insulate the electrical service drop under the scaffold and not providing a safe way on/off the scaffold.

29 CFR 1926.503(a)(1): The employer did not  provide a training program for each employee who might be exposed to fall hazards that enabled each employee to recognize the hazards of falling and that trained each employee in the procedures to be followed in order to minimize these hazards:   a) Job site, 1427 North Franklin Street, Pittsburgh, PA: On or about February 26, 2026 during box gutter restoration activities, the employer did not ensure an employee on a porch roof was properly trained in fall protection where fall hazards of approximately 10 feet existed.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways that enabled each employee to recognize hazards related to ladders and stairways, and that trained each employee in the procedures to be followed to minimize these hazards:   a) Job site, 1427 North Franklin Street, Pittsburgh, PA: On or about February 26, 2026 during box gutter restoration activities, the employer did not ensure an employee was trained on ladder safety prior to using a ladder to get on/off a roof where the ladder was only extended approximately 2 feet above the landing.
