29 CFR 1910.215(a)(4): Work rests on grinding machinery were not adjusted closely to the wheel with a maximum opening of 1/8 inch:  (a)NEXGEN US LBM dba Truss-Pro's - Location 2650, 10954 424th Avenue, Britton, SD 57430: On or about and at times prior to February 26, 2026, the employer did not ensure that the work rest on the DeWalt 6-inch Bench Grinder, Model Number DW756, Serial Number: 204C4YL within the Maintenance Room was adjusted to allow a max opening of 1/8 inch.  This condition exposed employees to struck by hazards associated with grinding wheel breakage.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch:  (a)NEXGEN US LBM dba Truss-Pro's - Location 2650, 10954 424th Avenue, Britton, SD 57430: On or about and at times prior to February 26, 2026, the employer did not ensure that the adjustable tongue on the DeWalt 6-inch Bench Grinder, Model Number DW756, Serial Number: 204C4YL being utilized within the Maintenance Room was adjusted to a distance not to exceed one-fourth inch.  This condition exposed employees to struck-by hazards associated with grinding wheel breakage.
