29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  (a) On or about February 27, 2026, an employee collecting debris and other materials around the perimeter of the building was not wearing a hard hat / protective helmet, while employees were working on the roofs above with tools and materials. This exposed the employees to struck-by hazards.

29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  (a) On or about February 27, 2026, the employer had not ensured that frequent jobsite inspections were conducted by a competent person(s) to identify and correct common safety hazards that employees were exposed to. Hazards identified during the inspection included but were not limited to, fall protection not being utilized while working at heights, hard hats for overhead hazards, and safety glasses not utilized when operating pneumatic nail guns.  Omega Roofing Limited Liability Company was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1801344, citation number 1, item number 1 and was affirmed as a final order on May 19, 2025, with respect to a workplace located at 830 Bridlewood Drive, Hartford, WI 53027.

(a) On or about February 27, 2026, the employer had not ensured employees were wearing safety glasses while installing shingles, exposing the employee to eye injury from flying objects, while using a pneumatic nail gun to install shingles. 29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about February 27, 2026, employees conducting roofing work on a new residential building with a 6:12 pitch, were not protected from falling to the lower level. This exposed employees performing roofing work on the property to fall hazards of approximately 12 feet on the lower roof and 30 feet on the upper roof to the ground below.
