On or about February 27, 2026, at an establishment located at 560 Jensen Road, Neenah, Wisconsin in the production area; the employer had not developed, document and utilize energy control procedures when employees were performing servicing and/or maintenance tasks, including but not limited to, cleaning and/or setup activities. The equipment had multiple energy sources, including but not limited to electrical, mechanical and pneumatic energy sources exposing employees to crushing and/or shear hazards.
