29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) On February 27, 2026, ladders used by employees to gain access to the working surface did not have side rails capable of extended at least 3 feet above the surface.  No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) On February 27, 2026, employees engaged in framing carpentry work to a newly built residential structure were not protected from fall hazards while working at elevations approximately 17.1 feet above the ground.  Veletyk Construction, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1690936, citation number 1, item number 1 and was affirmed as a final order on October 3, 2023, with respect to a workplace located at 28401 Torch Parkway, Warrenville, IL 60555.  No abatement certification or documentation is required for this item.
