29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation.  a) On February 27, 2026, at the above mentioned address, employees of Art Tyrawa dba DT Construction were exposed to eye injury when using an pneumatic nail gun to install roofing material.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(e)(1): Safe access was not provided to scaffold platforms when the platforms were more than 2 feet above or below the point of access:  a) On February 27, 2026, at the above mentioned address, the employees were exposed to fall hazards while accessing and egressing a ladder scaffold with a work platform approximately 22 feet above the ground.  In accordance with 29 CFR 1903.19 (d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstration that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1926.452(k)(1): Platforms shall not exceed a height of 20 feet (6.1 m).  a) On or about February 27, 2026, at the above mentioned address, the employees of Art Tyrawa dba DT Construction were working from a  ladder jack scaffold approximately 22 feet above the ground.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a)  On or about February 27, 2026, an extension ladder used by the employees to gain access to the working surface did not have its side rails extended 3 feet above the upper landing being accessed.
