Jobsite: On or about February 27, 2026, the employer did not ensure that the site foreman wore a hard hat when positioned under employees working above him, which exposed the site foreman to struck-by hazards.

29 CFR 1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm).  Jobsite: On or about February 27, 2026, the employer did not ensure that the scaffold planks on a supported tubular scaffold extended at least six inches over the centerline of the frame. The plank extended approximately three inches over the frame. This exposed employees who were using the scaffold as access and egress from the building to fall hazards of approximately 13 feet, one inch.

Jobsite: On or about February 27, 2026 the employer did not ensure that guardrails were installed along the open ends of the top level of a tubular frame scaffold.  Four employees who were working on the scaffold were exposed to fall hazards of approximately 13 feet, 1 inch from ground level.

29 CFR 1926.451(g)(4)(xv): Cross-bracing used in place of a mid-rail was unacceptable because the crossing point of two braces is between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a top-rail when the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a top-rail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform.  The endpoints at each upright were more than 48 inches (1.3 m) apart.  Jobsite: On or about February 27, 2026, the employer did not ensure that the second level of the scaffold system had a mid-rail installed at the bottom of the cross-brace. This exposed employees who were using the scaffold as access and egress from the building to fall hazards of approximately 13 feet, one inch.
