29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  (a) On or about 1/6/2026, on the east side of the jobsite, employees had access under and around scaffold that was cluttered with material, such as scaffold parts, wooden boards, and metal pieces to perform masonry related activities.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall:  (a) On or about 1/6/2026, on the east side of the jobsite, employees utilized a supported scaffold to perform masonry-related activities on the exterior of the building that did not have equipment in place to protect employees from falling objects. 29 CFR  1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program that included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):    (a) On or about 1/6/2026, the employer did not develop and maintain a written hazard communication program, even though employees throughout the site worked with hazardous materials, which included, but were not limited to, Quikrete Masonry and Mortar Cement, and bricks. The chemical hazards included, but were not limited to, skin irritation, serious eye damage, and respiratory irritation.
