29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a) On or about October 20, 2025, at Winter Boro High School, 22435 AL Hwy 21, Alpine, AL 35014, subcontractor's employees were not wearing head protection working under/below the ladder when employees were working on the roof.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  On or about October 20, 2025, at Winter Boro High School, 22435 AL Hwy 21, Alpine, AL 35014, subcontractor's employees were using an extension ladder that was not extended at least 3 feet over the upper landing.

29 CFR  1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  (a) On or about October 20, 2025, at  Winter Boro High School, 22435 AL Hwy 21, Alpine, AL 35014, the employer exposed employees to fall hazards, in that ladder training was not provided when employees were required to use a ladder to access the roof of the commercial building.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) On or about October 20, 2025, at Winter Boro High School, 22435 AL Hwy 21, Alpine, AL 35014: The employer did allow subcontractor's employees to perform roofing activities without fall protection.  The West Stillwell was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1659642, citation number 1, item number 1a and was affirmed as a final order on May 16, 2023, with respect to a workplace located at 4275Snow Rod, Semmes, AL 36575.
