29 CFR  1910.146(c)(7)(iii): The employer did not document the basis for determining that all hazards in a permit space have been eliminated, through a certification that contains the date, the location of the space, and the signature of the person making the determination. The certification was not made available to each employee entering the space or to that employee's authorized representative.  At 111 West St. Eunice Rd., Fulton, MO, 65251, On March 3, 2026 and at times prior thereto,  the employer did not document the basis for determining that all hazards in a permit space have been eliminated through a certification that contains the date, space location, and the signature of the person making the determination.  Employees enter the Simpson Mixer to perform cleaning and maintenance without substantiating all determinations that all mechanical hazards in the permit space have been eliminated as required by alternate entry provisions.

29 CFR  1910.147(c)(5)(ii)(B): Lockout and tagout devices were not standardized within the facility in at least one of the following criteria: color; shape; or size:  On or about March 3, 2026 at 111 West St. Eunice Road, Fulton, Missouri, 65251, employees utilized security locks as lockout devices that were not standardized thereby exposing them to hazards associated with the unexpected energization of equipment.

Procedures were not inspected annually to ensure that the procedure and the requirement sof the standard were met.

29 CFR  1910.303(b)(1)(viii): Electric equipment was not free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment was not determined using the following considerations: Other factors that contribute to the practical safeguarding of persons using or likely to come in contact with the equipment.  At 111 West St. Eunice Rd., Fulton, MO, 65251, on March 3, 2026 and at times prior thereto, employees were exposed to electrical hazards associated with a 480 volt panel and a shower wash station.   Employees were exposed to a shower wash station which was located above a 480 volt panel.  If activated the water from the shower would make contact with the panel causing an electrical hazard.
