29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about March 4, 2026, Wooster Products Inc. located at 3503 Old Airport Rd. Wooster, OH 44691, did not ensure machine specific procedures for the Cameron Slitter Model 6-A #1 were utilized prior to service and maintenance of the equipment took place. Operators are tasked to conduct set-ups and blade adjustments on the Cameron Slitter Model 6-A #1 , while energized exposing the employees to the unexpected energization or startup of the machine.  b) On or about March 4, 2026, Wooster Products Inc. located at 3503 Old Airport Rd. Wooster, OH 44691, did not ensure machine specific procedures for the Cameron Slitter Model 6-A #2 were utilized prior to service and maintenance of the equipment took place. Operators are tasked to conduct set-ups and blade adjustments on the Cameron Slitter Model 6-A #2, while energized exposing the employees to the unexpected energization or startup of the machine.

Wooster, OH 44691, did not ensure periodic inspections of energy control procedures were conducted at least annually, to confirm that the energy control procedures are adequate and properly implemented by authorized employees during servicing and maintenance activities.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about March 4, 2026, Wooster Products Inc. located at 3503 Old Airport Rd. Wooster, OH 44691, did not ensure that effective machine guarding was in place on the Cameron Slitter Model 6-A #1 machine. Guarding was not in place to protect employees from the rotating actions of the rollers. In-running nip points between the powered rollers, non-powered rollers, and the product itself, are present during operation of the machines exposing employees to caught-in hazards including but not limited to entanglement, crushing hazards, amputations, and degloving.  b) On or about March 4, 2026, Wooster Products Inc. located at 3503 Old Airport Rd. Wooster, OH 44691, did not ensure that effective machine guarding was in place on the Cameron Slitter Model 6-A #2 machine.
