29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) Assembly Department: On February 23, 2026, and at times prior, the employer exposed employees to amputation hazards, in that the guard over the cutting bit of the Swagelok Model TF16 tube facing tool (Serial No. TF1663822) was not sufficient to prevent the operator's fingers from entering the danger zone while they operated the machine to cut and shape metal tubes.
