29 CFR 1915.506(f): The employer did not ensure that the fixed extinguishing system was physically isolated before conducting maintenance. Summit Fire Protection Co. worksite located at 701 Maritime Drive in Ludington, MI on board the tugboat Undaunted: On or about March 2, 2026, the employer did not ensure the fixed carbon dioxide extinguishing system was physically isolated to control the hazardous release of carbon dioxide while employees performed annual maintenance and inspection activities in the bottle room of the tugboat, exposing employees to inhalation hazards.
