29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:   a) On or about March 4, 2026, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the back of the Accurshear 625010 shear was not guarded to prevent employee exposure to the crushing and/or shear hazards.   b) On or about March 5, 2026, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the unused portion of the HEM Sidewinder Auto horizontal band saw blade was not guarded to prevent employee exposure to the struck-by and amputation hazards.   c) On or about March 5, 2026, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the unused portion of the Roll-in EF band saw blade above the table was not guarded to prevent employee exposure to the shearing hazard.  d) On or about March 5, 2026, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the unused portion of the Roll-in EF band saw blade below the table was not guarded to prevent employee exposure to the saw blade during operation.

29 CFR  1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a) On or about March 5, 2026, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the Accurshear 625010 was not guarded to prevent access to the front holddowns when the shear was actuated. The gap between the front guard and the sheet metal measured 1/2 inch while permitting access within 1.5 inches of the shear holddowns.   b) On or about March 5, 2026, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the Mitsubishi LVPlus 3015 laser was not guarded to prevent access to the laser cutting area and/or exposure to visible and/or invisible laser radiation.   c) On or about March 5, 2026, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the Baileigh Industrial horizontal hydraulic press brake model HPB-45-NC was not guarded to prevent employee exposure to the point of operation when the foot pedal was utilized to actuate the press exposing employees to pinch point and/or crushing hazards.  d) On or about the week of February 23, 2026, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the LeBlond lathe was not equipped with a chuck guard exposing employees to entanglement hazards.   e) On or about March 5, 2026, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the Accurpress Press Brake 725012 was not equipped with point of operation guarding to prevent employee exposure to the downward motion of the ram creating crushing hazards when multiple bend parts were processed in the machine.   f) On or about March 5, 2026, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the Accurpress Press Brake 713010 was not equipped with point of operation guarding to prevent employee exposure to the downward motion of the ram.   g) On or about March 4, 2026, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the Mubea ironworker punch station was not provided with a complete guard to prevent employee exposure to the downward motion of the punch.

29 CFR  1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.  On or about March 5, 2026, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the pulley affixed to the electric motor on the Roll-in EF vertical band saw was not guarded to prevent employee exposure to in-running nip point hazards created against the rotating belt.

On or about March 5, 2026, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the belt affixed to the  pulley transmitting power from the electric motor on the Roll-in EF vertical band saw was not guarded to prevent employee exposure to in-running nip point hazards created against the rotating belt.

29 CFR  1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or reaccumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance.  On or about March 5, 2025, at a workplace located at 811 S Neenah Ave., Sturgeon Bay, Wisconsin; the employer did not develop, document and utilize energy control procedures when employees were performing servicing and/or maintenance tasks, including but not limited to, die and blade changes on equipment such as press brakes, shear, and saws. The equipment had multiple energy sources, including but not limited to electrical, mechanical, pneumatic, and/or hydraulic energy sources exposing employees to crushing and/or shear point hazards.

On or about March 5, 2026, at a workplace located at 811 S Neenah Ave., Wisconsin; employees permitted to perform die, tool and/or blade changes on the presses, shear, CNC, and saws were not trained as an authorized employee under the control of hazardous energy.
