The employer did not establish elements of a written respiratory protection program  to ensure that the employee had a medical evaluation to determine the employee's ability to use the respirator and training on the respirator cleaning, storage, and maintenance, on or about 3/05/2026.   b) Production area:  An employee used  3M half-face elastomeric respirator voluntarily and did not have a medical evaluation to determine the employee's ability to use the respirator, on or about 3/05/2026.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wore respirators when such use was not required by the employer:  a)Facility: Employees voluntarily wear N95 respirators to control airborne particulate hazards while assembling stained glass windows. The employer did not provide respirator users with the information contained in Appendix D to this standard when the employer determined that any voluntary respirator use was permissible, on or about  03/05/2026.
