29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed: a) For the Brightstar facility, On or about 03/06/2026, the employer exposed employees to amputation and electrical hazards, in that, there was no certification record of periodic inspections of energy control procedures. Machinery present included, but was not limited to, industrial printers with hazardous energy sources of 220/480VAC electrical, compressed air and moving parts.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) For the Tresu Flexo Innovator 1100-20 printer rollers, on or about 03/02/2026, the employer exposed employees to amputation hazards from ingoing roller nip points through which paper was fed, in that, roller nip points were not guarded to prevent employees from getting fingers caught-in.
