(a) Worksite - Conex Workshop: On or about 3/6/2026, the employer did not ensure that the cutting heads of the Ryobi under mounted table router was adequately guarded, which exposed employees to the point of operation while shaping starboard material. Employees operating the router in this condition were exposed to amputation type hazards.
