29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. a) Conexus Food Solution, Warehouse area: On or about March 6, 2026, employees were exposed to slip hazards in that the employer failed to maintain the walking-working surfaces in the freezer and refrigerated sections of the warehouse. Employees were required to work in areas where frozen ice and water had accumulated, creating hazardous conditions.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: a) Conexus Food Solution, Warehouse area: On or about March 6, 2026, employees were exposed to struck-by hazards in that the employer failed to ensure that shelving used for material storage was properly anchored to prevent accidental tip?over. Industrial truck manufacturing materials were not stored in a stable manner, and damaged storage racks were not secured against collapse.
