29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Job site, 4401 Ohio River Boulevard, Pittsburgh, PA: On or about March 6, 2026 during construction activities, the employer did not ensure employees used appropriate fall protection systems/devices where exposed to a fall hazard of up to approximately 9 1/2 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Job site, 4401 Ohio River Boulevard, Pittsburgh, PA: On or about March 6, 2026 during construction activities, the employer did not ensure a ladder used to get up/down to and from a lower roof was extended at least 3 feet above the landing.
