On or about March 9, 2026, at the above addressed jobsite, employees performing roofing operations at thirteen (13) feet five (5) inches above the ground were not protected from falls, thereby exposing employees to fall hazards. b. On or about March 9, 2026, at the above addressed jobsite, employees performing roofing operations at eight (8) feet two (2) inches above the ground were not protected from falls, thereby exposing employees to fall hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a. On or about March 9, 2026, at the above addressed jobsite, employees conducting roofing operations were not provided training on how to minimize fall hazards, thereby exposing the employees to the hazards associated with falls.
