This was observed on or about March 10, 2026, at 3001 N. Victory Road, Norfolk, Nebraska. Employee(s) were not utilizing fall protection when exposed to a fall of approximately 5 feet from the surface of flatbed trailers that were located inside of a structure.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Employee(s) were exposed to struck-by hazards. This was observed on or about March 10, 2026, at 3001 N. Victory Road, Norfolk, Nebraska. Employees were using an 8-inch Jet Bench Grinder and the Big Grinder that had the abrasive wheel periphery guards with 1-inch openings.
