29 CFR 1910.134(c)(1): The employer failed to have a written respiratory protection program in place for employees required to utilize respirators in the performance of their job tasks. Location - Worksite. On or about March 10, 2026, employees were required by the employer to wear N100 respirator mask while cutting brick. The employer had not implemented a written respiratory protection program before requiring the use of these respirators.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. Location - Worksite. On or about March 10, 2026, two employees were not provided a safe means of access to the working deck of a tubular welded scaffold.
