29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) Maintenance Area: On or about March 05, 2026 and at times prior, an employee used a bench grinder to grind damaged bolts where the work rest on the right side was not maintained within 1/8 inch of the abrasive wheel, exposing the employee to caught-in and amputation hazards. No abatement certification or documentation required.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) Maintenance Area: On or about March 5, 2026 and at times prior, an employee used a bench grinder to grind damaged bolts. The bench grinder was not equipped with a tongue guard on the abrasive wheel, exposing employees to struck-by and caught-in hazards.

29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid: a) 36 Phillips Lane, Fairmont, WV: On or about March 5, 2026 and at times prior, the employer did not have a person adequately trained to render first aid available at the facility, exposing employees to delayed medical treatment in the event of an injury or illness. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
