29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a) New residential construction located at Lot 546 Alice Way, Cumming, GA 30040, Third floor: On or about 03/11/2026, the employer exposed employees to fall hazards of approximately 21 feet to the lower ground level, in that a fall protection system was not provided employees engaged in framing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) New residential construction located on Lot 546 Alice Way, Cumming, GA 30040, Second floor: On or about 03/11/2026, the employer exposed employees to fall hazards at approximately 10 feet, to the lower ground level, in that the ladder used to access the second floor of the structure was not extended at least 3 feet above the upper landing surface.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  a) New residential construction located at Lot 546 Alice Way, Cumming, GA 30040: On or about 03/11/2026, the employer failed to inspect all electrical extension cords used to provide power to electric tools, in that an orange 50-ft. electrical cord was damaged and exposing the internal insulated energized wires. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.
