29 CFR 1910.29(f)(4): Openings in stair rails exceeded 19 inches (48 cm) at its least dimension: (a) Rail Debarker: On March 11, 2026 and times prior, an employee used the stairway to access the debarker machine. The stair rail system was not equipped with a mid rail resulting in an opening of approximately 36 inches between top hand rail and the bottom hand rail. No abatement certification or documentation required.
