29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)Hauger Construction, LLC at 224 Pheasant Ridge Drive Southeast, Watertown, SD 57201: On or about and at times prior to March 11, 2026, the employer did not ensure employees utilized protective head covers while exposed to hazards such as, but not limited to, unstable roof truss systems, rigging equipment, and tools.  This condition exposed employees to struck-by hazards while working directly adjacent to and below roof truss framing activities.

This condition exposed the employees to fall hazards greater than six feet.  (b)Hauger Construction, LLC at 224 Pheasant Ridge Drive Southeast, Watertown, SD 57201: On or about and at times prior to March 11, 2026, the employer did not ensure that an employee was protected from falls from a framed mezzanine while conducting framing activities on a commercial building. This condition exposed the employees to fall hazards greater than six feet.

This condition exposed employees to fall hazards greater than ten feet.  (b)Hauger Construction, LLC at 224 Pheasant Ridge Drive Southeast, Watertown, SD 57201: On or about and at times prior to March 11, 2026, the employer did not ensure that scaffolding training was conducted for the employees who were using the JLG Scissor Lift, Model Number: 260MRT,  Serial Number: C200149449 prior to conducting framing activities on a commercial building.  This condition exposed employees to fall hazards greater than ten feet.
