29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring systems:  a) Jobsite, 1000 Wingate Drive, Pittsburgh, PA:  On or about March 3, 2026, two employees were installing metal sheet panels on a commercial roof at heights of 46-54 feet above the ground without utilizing fall protection.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) Jobsite, 1000 Wingate Drive, Pittsburgh, PA:  On or about March 3, 2026, employees work at heights performing roofing activities and had been trained in fall protection by the owner but the owner did not have any documentation of it.
