Employees were engaged in a size change on the toggle press without all hazardous energies present on the equipment isolated in accordance with the procedures.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about March 11 2026, the employer did not ensure that a periodic (annual) inspection of the energy control procedures for equipment had taken place at the facility. Equipment present at the facility includes but is not limited to presses. Hazardous energies present on equipment includes but is not limited to gravitational, kinetic, pneumatic, mechanical, and electrical.
