29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. Ground outside Suite 310 - On or about March 13, 2026, employees working on the ground cleaning up debris being thrown down and falling from the roof of the building were subject to struck by falling material hazards without head protection.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest.   a. The Roof - On or about March 13, 2026, employees were engaged in shingle removal and replacement on the top of a tiered roof which included a low slope section of roof without fall protection.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  a. The Roof - On or about March 13, 2026, employees were using body belt rock climbing harnesses with a front attachment point while performing shingle removal and replacement activities over 15 feet above the paved concrete sidewalk. Employees were exposed to a fall hazard.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
