29 CFR 1926.451(g)(1):  Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) Single-family home exterior, east side; On or about March 12, 2026, an employee working from a pump jack scaffold system to remove siding panels was not protected from fall hazards of approximately 14-feet or more above ground-level as the scaffold was not equipped with a mid-rail, screen or mesh along the length of the work platform, end rails were not in place, and alternative fall protection system(s) were not in use.  (b) Single-family home exterior, north side; On or about March 12, 2026, an employee working from a pump jack scaffold system to remove siding panels was not protected from fall hazards of approximately 10-feet, 10-inches above ground-level as the scaffold was not equipped with a mid-rail, screen or mesh along the length of the work platform, end rails were not in place, and alternative fall protection system(s) were not in use.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Single-family home exterior, east side; On or about March 12, 2026, employees working from the sloped lower rooftop to remove siding panels or walking along the roof to access an adjacent scaffold system were exposed to an approximate 9.5-foot fall hazard presented by the unprotected roof eave and personal fall arrest systems, or  alternative fall protection method(s), were not in place.

29 CFR 1926.1053(b)(9): The area around the top and bottom of ladders was not kept clear:   (a) Single-family home exterior, east side ground-level; On or about March 12, 2026, material consisting of steel siding scrap and construction debris associated with siding replacement work was not kept clear from the area immediately around the base of an extension ladder, which was used to access and egress the lower rooftop.
