29 CFR  1926.451(c)(2)(iv):  Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:  On or about March 13, 2026, while installing framing on a residential new build located at 7747 Harley Hills Road, North Royalton OH; the employer failed to ensure that employees working from elevated heights of approximately 15 feet by standing on a pallet which was attached to the raised fork of a CAT JLG telehandler, were protected from fall hazards, in that they and the employer himself were actively working from the elevated pallet without any fall protections in place.

29 CFR  1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:  On or about March 13, 2026, while installing framing on a residential new build located at 7747 Harley Hills Road, North Royalton OH; the employer failed to ensure that the portable ladders employees were using to access the upper window above the garage, were placed on stable and level ground prior to the employees using them.  Employees were exposed to a fall from height of approximately 21.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about March13, 2026, while installing framing on a residential new build located at 7747 Harley Hills Road, North Royalton OH; the employer failed to ensure that employees working from elevated heights were protected from fall hazards of approximately 10-30 feet at varying levels.  The employer, Larson's Corner Construction, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1737713, citation number 1 item number 1 and was affirmed as a final order on August 1, 2024, with respect to a workplace located at 102 Rames Gate Rd., Cranberry Twps., PA 16066.  The employer, Larson's Corner Construction, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1516892, citation number 1 item number 2 and was affirmed as a final order on April 13, 2021, with respect to a workplace located at 1769 Abbeyville Rd., Valley View OH 44280.
