29 CFR  1910.266(d)(1)(vi): The employer did not assure that each employee who worked in an area where there was potential for head injury from falling or flying objects wore head protection meeting the requirements of subpart I of Part 1910:  (a) At the boom boat haul out site of the Viking Lumber log transfer facility (LTF) in Naukati Bay, AK, on or about February 13, 2026, and at times prior thereto, workers were exposed to injury, in that hard hats were not being worn when there was a potential for head injury.
