OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck?by hazards due to the failure to secure an industrial steel storage rack to the floor. Location: High Bay area: On or about March 5, 2026, industrial steel storage racks located in the high bay were not anchored or bolted to the concrete floor as required by industry?recognized practices, exposing employees to potential struck by or crushed by hazards. The unanchored racks were loaded with stored materials and presented a recognized hazard of tipping, shifting, or collapse when subjected to routine forces such as forklift contact, vibration from material?handling traffic, or accidental impact.

29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), was dispensed into containers unless the nozzle and container are electrically interconnected. Location: High bay area. On or about February 4, 2026, employees were exposed to fire and explosion hazards while transferring JP-5 and Jet A-1 fuel from 55 gallon drums with an air-operated ball syphon where the source drum, flammable storage container, syphon device and receiving containers were not bonded or grounded.
