Location- 260 Landham Rd, Sudbury, MA  On March 15, 2026, employees were exposed to fall hazards of approximately 25 feet while working from a scaffold on a residential construction project where a competent person qualified in scaffold erection was not present to supervise or direct the scaffold erection

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Location: 260 Landham Ed, Sudbury, MA  Instance 1- On March 15, 2026, workers were exposed to fall hazards of up to 25 feet to the ground below while conducting framing activities where fall protection was not used  Instance 2- On March 15, 2026, workers were exposed to fall hazards of up to 10 feet to a lower landing surface when an open-sided walking surface (stairwell) was not guarded

Fall protection training was not provided to recognize and minimize fall hazards.
