29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.  a) Next to the exit door by Modification and Staging area: On or about March 16, 2026, portable fire extinguishers were not clearly identified or marked, which could delay locating and accessing them during a fire emergency. b) Next to the entrance door of the warehouse: On or about March 16, 2026, locations of portable fire extinguishers were not clearly identified or marked, potentially delaying access during a fire emergency. c) On the back wall by the staging area: On or about March 16, 2026, a portable fire extinguisher located on the back wall near the staging area was blocked by pallets of cardboard, preventing proper access to the extinguisher in the event of a fire.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  a) Aisle A3: On or about March 16, 2026, materials stored at ground level were not properly stacked or secured. Several upper pallets extended beyond the lower pallets, creating the potential for pallets to slide and resulting in struck?by and trip hazards.  b) Aisle AZ: On or about March 16, 2026, the employer failed to ensure that materials stored at ground level were properly stacked and secured. Several upper pallets extended over the lower pallets, creating the potential for pallets to slide and resulting in struck?by and tripping hazards.
