29 CFR  1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.  On or about March 6, 2026, the employer failed to ensure the two-hand control guarding device that lowered the front door guard of the Auto Sandblaster No. 1 Machine was such that it did not offer an accident in itself when the machine was operated with multiple operators.  Only one two-hand control guarding device was provided that permitted a second machine operator to be within the hazard area created by the closing of the front door guard when the two-hand control guard device was cycled by another operator.
