29 CFR  1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:   51 Grove Ave, Leominster, MA:  On March 19, 2026,  employees were exposed to a 24 foot fall hazard when they accessed a ladder jack scaffold platform that was more than 2 feet above the point of access.

29 CFR  1926.451(f)(6):  Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, were closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).  The minimum allowable distance for any level of voltage listed on the table is three feet:  51 Grove Ave, Leominster, MA:  On March 19, 2026, employees erected and worked from an all-metal ladder jack scaffold with one of the of the metal supporting ladders placed within two feet from the building's energized electrical service drop wire.

29 CFR  1926.451(f)(7):  Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration.  Such activities were not performed only by experienced and trained employees selected for such work by the competent person:  51 Grove Ave, Leominster, MA:  On March 19, 2026, employees who erected ladder jack scaffolds were inexperienced, untrained, and not supervised by a competent person.  The lack of training and supervision resulted in scaffold erection deficiencies and their exposure to electrical hazards.

Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not immediately marked in a manner that readily identified them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  51 Grove Ave, Leominster, MA:  On March 19, 2026, employees worked from ladder jack scaffolds that had a supporting ladder with one supporting foot that was bent and had the access ladder with two bent rungs.

The lack of training resulted in employees working from a scaffold with multiple unrecognized hazards that were created during the scaffold erection.
