29 CFR  1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  a.) Ultron Silicone Coating Line - On or about March 16, 2026,  a line operator was injured while cleaning the gravure roll on the Ultron Silicone Coating Line when the employees right index finger became caught between the gravure and application rollers, resulting in a fingertip amputation.  The employer failed to provide adequate training for employees performing service and/or maintenance activities to ensure the knowledge and skills required for the safe application, usage and removal of energy control devices were acquired.   b. Ultron Silicone Coating Line - On or about March 20, 2026, a line operator performed the service and maintenance task of cleaning a roller on the Ultron Silicone Coating Line.  The employer failed to provide adequate training for employees performing service and/or maintenance activities to ensure the knowledge and skills required for the safe application, usage and removal of energy control devices were acquired.  c. Goebel 1 Slitter - On or about March 25, 2026, a supervisor performed the service and maintenance task of replacing bearings on the Goebel 1 Slitter.  The employer failed to provide adequate training for employees performing service and/or maintenance activities to ensure the knowledge and skills required for the safe application, usage and removal of energy control devices were acquired.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a.) Ultron Silicone Coating Line - On or about March 16, 2026, a line operator was injured while cleaning the gravure roll on the Ultron Silicone Coating Line when the employee's right index finger became caught between the gravure and application rollers, resulting in a fingertip amputation.  The employer failed to implement energy control application steps as the machine was not shut down or turn off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

Production - On or about March 25, 2026, the employer failed to ensure that employee(s) who operate forklifts were properly trained, evaluated and certified to perform the task in a safe manner, thereby exposing employee to struck-by hazards.  An employee used a Toyota forklift, Model 7FGGU25, to move a roll of paper to the Ultron 22" Silicone Coating Line.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a. Production - On or about March 25, 2026, the employer failed to provide effective guarding to protect employees working at or near the Ultron 22" Silicone Coater Line machine.  The right brake guard was missing guarding device for Ultron 22" Silicone Coater Line, did not prevent employees from operating the machine while in the zone of danger during the bending process.  Employees were, thereby, exposed to a rotating parts and amputation hazards.  b. Production - On or about March 25, 2026, the employer failed to provide effective guarding to protect employees working at or near the Leeson Electric Motor, Model C4D17FK5L to prevent employees from operating the machine while in the zone of danger during the bending process.  Employees were, thereby, exposed to a rotating parts and amputation hazards.

Production Floor - On or about March 25, 2026, the employer did not ensure there was sufficient space to access the electric panels.  The panels were blocked by two hand trucks, a desk, chair, and a Wet-Dry vac machine, thereby exposing employees to trip and fall hazards associated with blocked and/or obstructed electrical panels.

Tacknologies, Ltd. located at 10720 Beaudin Boulevard, Suite A, Woodridge, IL 60517 - On or about March 16, 2026, an employee sustained a right index fingertip amputation while cleaning the gravure roll on the Ultron 22" Silicone Coater Line.  The employer did not record the injury on the OSHA 300 Form and OSHA 301 Injury and Illness Incident Report, or an equivalent form, within seven (7) calendar days.
