29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns were not protected by protective helmets: (a) On or about March 20, 2026, an employee was on the ground cutting siding on the ground without a hard hat on, while other employees were working from a pumpjack scaffolding installing siding and throwing debris from the scaffold above to the ground below, exposing the employee to a struck-by hazard.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level. (a) On or about March 20, 2026, employees were installing siding, on a new residential property, from a pumpjack scaffold system without a proper fall protection system in place, exposing employees to a 18 1/2 foot fall hazard.
