29 CFR  1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a. On or about March 19, 2026, at the jobsite: the employer allowed employees to work in an excavation without a stairway, ladder, ramp or other safe means of egress located in an excavation 14' long x 29' wide and 5'8" deep with near vertical walls, in class c soil, exposing employees to cave-in and suffocation hazards.

29 CFR  1926.651(i)(2)(i): A support system, such as underpinning, was not provided to ensure the safety of employees and the stability of the structure.  a. On or about March 19, 2026, at the jobsite: employees were exposed to fall and struck-by hazards, in that a slab of concrete of the parking lot was not supported or underpinned while employees stand and worked around it installing sewer pipe lines.

On or about March 19, 2026, at the jobsite: the employer allowed employees to work in an excavation without the excavated materials at least 2 feet from the edge of an excavation 14' long x 29' wide and 5'8" deep, with near vertical walls, in class C soil, exposing employees to crushing and struck by hazards.

29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a. On or about March 19, 2026, at the jobsite: employees exposed to cave-in, suffocation and struck-by hazards, in that a cave-in protection was not installed in a 5'8" foot deep trench, with near vertical wall in type c soil, while contractors and subcontractors were installing an E1 Station and 6" SDR46 sewer pipes.
