29 CFR  1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met, and had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):    a)	2 Manor Blvd, Mifflintown, PA 17059:  On or about March 24, 2026, employees utilized cleaners, sanitizers, and laundry detergents, and the employer had a written hazard communication program, however the program was deficient in that the program did not include: a.	How the safety data sheets are to be maintained and how employees obtain access to the SDSs. b.	Procedure(s) to follow to determine if the SDS is current. c.	The designation of the person responsible for conducting the training. d.	The description of labeling system(s) used, and /or the description of alternative to labeling for workplace containers.  b)	2 Manor Blvd, Mifflintown, PA 17059:  On or about March 24, 2026, employees utilized cleaners, sanitizers, and laundry detergents.  The employer did not compile a complete list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet in the workplace.  The chemicals that were not listed included, but were not limited to: a.	Peroxide Multi-Surface Disinfectant and Cleaner b.	Cleanslate Yuma Bathroom Cleaner c.	Cleanslate Glassboro Window and Glass Cleaner  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  a) 2 Manor Blvd, Mifflintown, PA 17059 - Wing 2 Housekeeping Closet: On or about March 24, 2026, employees were exposed to chemical hazards due to an unlabeled secondary container (spray bottle) which contained a purple liquid.  The container did not include the product identifier and hazard information.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1904.29(b)(1):  The employer did not provide accurate information on the OSHA Form 300 and/or 300A, or equivalent.  a) 2 Manor Blvd, Mifflintown, PA 17059, on or about February 6, 2026, Column F for Case #1 on the 2026 OSHA Form 300, Log of Work-Related Injuries and Illnesse, did not describe the object/substance that directly injured the employee.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
