29 CFR 1910.178(n)(4): The driver was not required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing: On or about March 17, 2026, and March 24, 2026, employees in the WIP staging area were exposed to struck-by injuries because the employer failed to ensure that employees operating powered industrial vehicles were sounding the horn at cross aisles and other locations where the vision was obstructed.
