29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  At the temporary jobsite located at 3427 Thunder Wichita, KS 67219, employees performing roofing activities were exposed to fall hazards.  Employees were exposed in that the employer had not provided training or instructions to employees regarding the recognition and avoidance of unsafe conditions associated with construction work and the methods to mitigate those hazards, including but not limited to: how and when to use fall protection equipment, safety glasses, hard hats, how to set up and use ladders properly, and hazard communication training.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At the temporary jobsite located at 3427 Thunder Wichita, KS 67219, employees performing roofing activities on a 6:12 pitched roof were exposed to fall hazards of approximately 9 feet 1 inch from the roof to the ground below in that fall protection was not utilized.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the temporary jobsite located at 3427 Thunder Wichita, KS 67219, employees performing roofing activities were exposed to fall hazards greater than 6 feet in that the ladder used to access and egress the roof was not extended three feet above the eave or was not secured to the structure.
