29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: At the temporary jobsite located at 614 Thoroughbred Ct. Wichita, KS 67235, employees engaged in roofing activities were exposed to fall hazards of approximately 11.6 feet from the eave of the roof to the ground below in that the ladder used to access and egress the roof was not extended three feet above the eave or was not secured to the structure.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At the temporary jobsite located at 614 Thoroughbred Ct. Wichita, KS 67235, employees engaged in residential roofing activities on a 10:12 pitched roof were exposed to fall hazards of approximately 14.5 feet from the eave of the roof to the ground below in that fall protection was not utilized. Gregorio Diaz dba Goyos Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1597235, citation number 1, item number 1 and was affirmed as a final order on 06-22-2022, with respect to a workplace located at 1626 Lantern Lane Ct. Andover, KS 67002.
