Midwest Hose and Specialty, Inc. dba Midwest Hose LayFlat Division - On or about 3/24/26, and at times prior thereto, evaluations of forklift operators were not being conducted every three years.  Employees operate forklifts including, but not limited to, Toyota Forklift, Model: 8FGCU30, Serial No: C3258.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
