Located at Lot 35, Crescent View Drive, Canal Winchester, OH: On or about March 23, 2026, employees were installing vinyl siding from three pump jack scaffolds. The scaffold poles were placed on unlevel and unstable ground conditions consisting of wet, muddy soil. As a result, employees were exposed to fall hazards due to the instability of the scaffold platforms.

Multiple scaffold poles were in use that were not plumb and effectively supported/braced. As a result, employees were exposed to fall hazards due to the instability of the scaffold platforms.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. a. Located at Lot 35, Crescent View Drive, Canal Winchester, OH: On or about March 23, 2026, employees were installing vinyl siding from three pump jack scaffolds. The scaffold poles were placed on unlevel and unstable ground conditions, multiple poles were not plumb and level, and incomplete fall protection systems were in place. The employer did not ensure that a competent person was onsite and performed an inspection of the scaffold to identify unsafe conditions. As a result, employees were exposed to fall hazards.

Located at Lot 35, Crescent View Drive, Canal Winchester, OH: On or about March 23, 2026, employees were installing vinyl siding from pump jack scaffolds. The employer did not ensure that employees were protected from falling from the open and unprotected ends of the scaffold platforms that were elevated approximately 11' to 20' from the ground. As a result, employees were exposed to fall hazards from the platforms while working at the ends.
