29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) New residential construction located on Lot 67 in the Clark Farms subdivision, Flowery Branch, GA, 30542, ground level: On or about 3/24/2026 the employer exposed employees to struck-by hazards while working below overhead decking operations, in that protective headwear was not provided to the employees on the ground.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  a) New residential construction located at Lot 67 in the Clark Farms subdivision, Flowery Branch, GA, 30542, second level: On or about 3/24/2026, the employer exposed employees to struck-by hazards while operating pneumatic nail guns, in that protective eyewear was not provided to employees using these tools at 90 PSI while engaged in framing activities.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New residential construction located at Lot 67 in the Clark Farms subdivision, Flowery Branch, GA, 30542, second floor: On or about 3/24/2026 the employer exposed employees to fall hazards of 10-feet, 7-inches to the lower level, in that a fall protection system was not provided to workers performing framing and plywood sheathing installation activities.
