29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a.)On or about March 17, 2026, the employer failed to ensure employees were protected from accessible moving machine parts such as the roller chain edge on the conveyor system at Line 104. Employees could access the ingoing nip point between the conveyor and the oven where an injury occurred. As a result, employees were exposed to caught?in hazards created by rotating parts and ingoing nip points. b.)On or about March 25, 2026, the employer failed to ensure employees were protected from accessible moving machine parts on the conveyor system at Line 104. Employees could reach through an approximate 7.5?inch opening at the char marker on the conveyor where an exposed rolling sprocket was located.

29 CFR 1904.32(a)(1): Basic requirement. At the end of each calendar year, you must: Review the OSHA 300 Log to verify that the entries are complete and accurate, and correct any deficiencies identified; a) On or about December 31, 2025, the employer failed to review the OSHA 300 Log and OSHA 300?A to verify that each entry for the year was complete and accurate and to correct deficiencies, resulting in incomplete injury and illness descriptions. As a result section F of the logs contained description which noted only the general body part affected, omitting critical information such as the object or substance causing the injury and a descriptive account of how it occurred.
